Immigration Physicals Offered at Centennial

Rocky Mountain Family Medicine is pleased to announce that it is now offering immigration physicals at its Centennial, CO office. The cost of an immigration physical is $150.00. Blood/laboratory testing and vaccinations will be at an additional charge. Please call 303-693-2000 for an appointment. Please download a copy of Form I-693 and complete the patient portion. Bring this form with you to the appointment. Please also bring a copy of your vaccination records with you to your appointment. You may obtain a list of the required vaccinations from the CDC's web site at http://www.cdc.gov/immigrantrefugeehealth/exams/diseases-vaccines-included.html
