Discount Health Plan
Rocky Mountain Urgent Care offers a Discount Health Plan to patients and their families without medical insurance as well as those insured patients with high deductibles and lapse in coverage. With proof of membership in the plan a patient is able to be seen for services at any Rocky Mountain Urgent Care and Family Medicine facility for a reduced fee, including family practice, urgent care, psychology and physical therapy services.
The Discount Health Plan is valid for only family practice and urgent care provider services provided by Rocky Mountain Urgent Care and Family Medicine. It is also valid for psychology or physical therapy services. In addition, members will receive discount vision, dental, physician/hospital, and pharmacy benefits throughout Colorado.

To become a member of our Discount Health Plan, you can stop by any Rocky Mountain Urgent Care and Family Medicine location to pick up an application or you can print out the application below and fill it out before your visit.

Cost
As a member of our discount program you and your family members will receive regular urgent care visits for a low flat fee, as well as a 36% discount off of our prices on all other urgent care services provided by our urgent care group. Family practice, psychology and physical therapy services are offered at a 20% discount off of our prices.
The cost of membership is $18 a month, in addition to a one-time $5 application fee.
The Discount Health Plan includes a spouse and all legal dependents. It is NOT insurance; it is a discount card that can be used for discounts on Rocky Mountain Urgent Care and Family Medicine services, other physicians, hospitals, dental and vision, as well as local pharmacy and mail-order prescriptions.

Proof of Membership
In order to use the Discount Health Plan, you must present your Discount Health Plan Identification Card, along with valid photo identification at the time medical service is rendered.
Services NOT included:
- Occupational and Workers Compensation Services
- Labs performed outside our facility
- Radiology Services performed in our facility but by a different entity
Return Visits for the Same Problem
If a patient returns to any Rocky Mountain Urgent Care for the same problem, within seven (7) days of the initial office visit, the cost to see the provider will be $36.00 for a regular office visit. All other services will continue to be discounted at 36%.
Cancellation Policy
The Discount Health Plan may be cancelled with a full refund of Health fees (excluding the application fee) within the first 30days of enrollment.
