• Contact Member Services at Galaxy Health Network by calling 800-975-3322 
• A Patient Advocate will contact providers specific to your zip code, physicians/facility/and/or specialty being requested. They will also verify that the physician/facility understands that they are part of the Galaxy Health Network, and confirm they will accept they Physician Visit Card. (Patient Advocacy not available in California). 
• Call to make an appointment and pay the entire amount at the time of service. 
• You must show your Galaxy Physician Visit Health card to the Provider at the time of the visit to receive the discount. That amount will be reflected on your bill. Payment is due at the time services are rendered. 
• Discounts apply only to services performed within the physician’s office. 

If your favorite physician is not already a participating provider, call Member Services to nominate him or her. Have the physician’s name, address, and telephone number available when you call.
