Rocky Mountain Urgent Care and Family Medicine offers discounted rates to all private pay patients. We also have a discount membership plan available for patients and their families without medical insurance as well as those insured patients with high deductibles or lapse in coverage. With proof of membership, a patient can be seen at any Rocky Mountain Urgent Care and Family Medicine facility for a reduced fee.

The Discount Membership Plan is valid for all provider services provided by Rocky Mountain Urgent Care and Family Medicine.  In addition, members will receive discount vision, dental, physician/hospital, and pharmacy benefits throughout Colorado.

Program Details

Cost - As a member of our discount program you and your family members will receive regular office visits for $92 and office visits with procedures for $150.  The membership cost is $54/quarter plus a one time $5 application fee. 

Membership - the discount membership plan includes spouse and all legal dependents.  It is NOT insurance; it is a discount card that can be used for Rocky Mountain Urgent Care and Family Medicine, Other Physicians, Hospitals, Dental, Vision, and Local Pharmacy and Mail Order services.

Proof of Membership - In order to use your Discount Membership, you must present your Discount Membership Plan Identification Card, along with valid photo identification at the time medical service in rendered.

Services NOT included:

• TB Testing
• Occupational and Workers Compensation Services
• Vaccines including Tetanus
• Labs performed outside our facility

Return Visits for the Same Problem - If a patient returns to any Rocky Mountain Urgent Care and Family Medicine for the same problem, within seven (7) days of the initial office visit, the cost to see the provider will be $46.00 for a regular office visit and $75.00 for an office visit with a procedure.

Cancellation Policy - The Discount Membership Plan may be cancelled with a full refund of membership fees (excluding the application fee) within the first 60 days of enrollment. However, if you have purchased your membership during a Rocky Mountain Urgent Care and Family Medicine visit and you give notice of cancellation within the first 60 days, your refund will be less the discount given at the time of service. This situation may require you to pay back  a portion of the discount you received at time of service.

Sign Up - To become a member of our Discount Plan you can stop by any Rocky Mountain Urgent Care and Family Medicine location to pick up an application or you can print out an online application and fill it out before your visit.
