Planning Your Visit to Family Medicine

You will need to bring the following documents with you to your visit:

• Picture Identification (drivers license)
• Insurance Card
• Co-pay (We accept cash, checks, and all major credit cards)
• For Minors (non-emancipated children under the age of 18), we require a written authorization for treatment from a legal guardian.
