You will need to bring the following documents with you to your visit:
- Picture Identification (for children we will need a parental photo i.d.)
- Insurance Card
- Copay (We accept cash, checks, and all major credit cards)
For Minors (non-emancipated children under the age of 18), we require a written authorization for treatment from a legal guardian.
